Virtual Outcomes – GP Surgery Module

Virtual Outcomes have launched a module to support pharmacies in implementing the Hypertension Case Finding service with your GP Surgeries.  The module can be accessed here:

This link can be shared with your surgeries, and can be used to help facilitate the PCN-wide discussion on:

• Which patients the surgeries may refer to pharmacy
• How this referral from surgery to pharmacy will happen
• Where the Out Of Hours referrals for urgent same day cases will be referred to if GP surgery can’t see patient that day.
