Series 4 smartcard replacement

Old series 4 smartcard need to be replaced before 30th December 2021
NHS Digital is upgrading the Smartcard system and software. A small percentage of cards will reach end-of-life and are scheduled to no longer be supported after 30th December 2021. Users of older cards should replace their cards well ahead of the end-of-life deadlines.
Check if your card needs replacing
Series 4 Smartcards do not have a blue band across the top.

What do you need to do?
Contact the Registration Authority (RA) by email and include:
- Contact details
- Smartcard information
- The following text ‘Please can I request you issue me with a new Smartcard, given my card is reaching end-of-life’
