PQS: Palliative and End of Life Care Domain

None of the locally commissioned services across Hampshire & Isle of Wight ICS and Frimley ICS contain all 16 of the national list of drugs. The drugs stocked locally are what OUR prescribers use.

All pharmacies who intend to complete this domain for their PQS do not need to stock the meds but must have an action plan.

The action plan must include:

• Details of any locally commissioned service pharmacies nearby to refer patients and carers to;
• All our locally commissioned pharmacies stock haloperidol injection as part of the service;
  • Make a note of this in your action plan
• Whether any other local pharmacies have declared yes to stocking the 16 critical medicines on the Directory of Service (DoS) and how to access the Service Finder to view them.
