Anticoagulation Audits, MYS now available for data entry
For pharmacies that have already completed or are currently doing their Anticoagulation audit, MYS is now available to record the results of the audit. MYS will stay available until the end of the declaration window in February 2022.
For those that haven’t started yet, the domain requirements are:
- The audit must be carried out over two weeks (extended to four weeks if less than 15 patients were seen during the first two weeks).
- A minimum of 15 patients on DOACs to be audited.
- Any patient with concerns must be referred to their GP and you must follow up to find out outcomes and identify any actions taken (be aware this follow up may occur after the declaration window has closed).
- Anonymised data need to be entered on MYS - PQS Oral Anticoagulant Safety Audit 2021/22 tab (don’t forget to transfer if you have used the paper audit form).
- Keep a note of the dates the audit was conducted between for declaration.
