PharmOutcomes information update: Transfer of TCAM to Discharge Medicines Service

Following the publication of the Discharge Medicines Service (DMS) Toolkit, Pinnacle Health have now shared their plans for the further development of their ‘PharmOutcomes’ platform to support the introduction of the DMS essential service going forward
