Local Patient Safety Error

Oramorph Concentrated Oral Solution 20 mg/ml
Recently a GP surgery inadvertently prescribed Oramorph concentrate liquid when they meant to prescribe ‘normal’ oramorph liquid.
There has been an ask of pharmacy by surgeries and the ICB.
If you get a prescription for oramorph concentrated liquid, it is asked that you check the patient’s national care record/ CHIE/ or with the GP surgery to check they haven’t made a mistake when selecting the drug they have prescribed especially if it is a first script.
Do not rely on the patient to confirm the strength is correct.
