Hypertension Case Finding Service – PharmOutcomes

Service now available on PharmOutcomes to record the consultation
All pharmacies who have registered as live on MYS have now been accredited to the Hypertension case finding service templates on PharmOutcomes.
The three modules are now available to pharmacies on the services screen under the advanced service section towards the top of their list on the left hand side as shown in image below.
The benefits of using the PharmOutcomes module:
- Results sent directly to GP surgery on consultation completion*
- Supports completion on MYS for claiming
- Stores consultations electronically
The service is split into 3 stages:
- Hypertension case finding (AS) – Records the consultation and readings and fitting of an ABPM if suitable on the day of readings.
- Hypertension case finding ABPM Fitting – When fitting is not suitable on the day the patient can return and the consultation recorded here.
- Hypertension case finding ABPM follow-up – Where an ABPM was fitted and the patient returns to store, the readings and follow up consultation is recorded here.
For new pharmacies signing up there will be a short delay whilst the service is accredited to you.
