Discharge Medicines Service (DMS): Essential Service Reminder

A reminder to all pharmacies that the Discharge Medicine Service is an Essential service that ALL pharmacies must deliver. Recent completion data shows a concerning gap between those being referral and those being completed.  

• Hampshire Hospital Foundation Trust, HHFT (Winchester & Basingstoke)                51%
• St Mary’s (Isle of Wight)                                                                                              71%
• University Hospital Southampton, UHS (Southampton)                                              55%

At present only pharmacies with referrals from St Mary’s Isle of Wight have an acceptable completion rate. Please remember, since last year, pharmacies are now paid to provide this service.
